 Could be many reasons:
1. Other substances consumed
2. Nervous becasue he may be fucking another guys wife...while guy is watching
3. Not that attracted female (unexpected muff or smell or wart)

 As one who experienced this, I was disappointed in myself and embarrassed. This was at a swing party and couple swap. I made it a point of spending a good amount of time with my partner,fingers and tongue until she came a couple of times (don't think she was faking it). Didn't understand it myself and it wasn't because she was unattractive, etc. Can't tell you why it happened then or why it has happened other times. I'm not a drinker, so alcohol wasn't the issue!
